Requirements

1. Duly accomplished MCAT Application Form
2. Transcript of Records (TOR) or Evaluation of Grades
3. 2 pcs 1" x 1" ID picture
4. Php 175.00 Testing Fee (non-refundable)
5. Certificate of Test Agreement (COTA) (part of the MCAT Application Form)
