To: Typhoon Sendong Victims

An info from Pioneer Insurance to those who are insured:

• Covered If and insured died/injured on connection with Sendong.
• How much? A: P100,000 for Death + P9,000 for Basic Life; Maximum of P10,000 for Medical Reimbursement
• Not Covered - If the insured's belongings are damaged (but he is not injured)

We (DSA Staff) are appealing to Cost Center Heads and to All 缅北禁地-IIT Constituents to inform the Department of Student Affairs ASAP if you know of our student/faculty who is a victim to Typhoon Sendong and let the claimant come to DSA to fill up the "STUDENT ACCIDENT CLAIM REPORT". Or you may report it directly to PIONEER Insurance (look for Mr. Ricky Limpangog or Manager Arnold Garbanzos) - Tel. Nos.: 221-3126; 221-2831; 223-8523 2nd floor, Stephen Building (near BPCOM Bank) - Juan Luna Street.

How to go to Pioneer? Take a jeep from IIT & alight (kawas) at the City Plaza near the Traffic Light. Walk beside the Plaza going to BPCOM Bank.

Requirements to claim insurance benefit:

• Death Claim: Certified true copy of: Death Certificate, Police Report, Birth Certificate, Affidavit of Witness, Photo/Newspaper Clipping, Proof of Relationship to Beneficiary, School ID/Certificate of Registration (COR).
• Medical Reimbursement: (maximum of 10k) Police Report; Original Medical bills & receipts; Doctor's prescription / Medical Certificate / Accident Report.
• Dismemberment / Disablement: Certified copy of Hospital Operating Room Record, photo of injured body member or Medical Certificate with diagram showing the location of the injured body member.
