1. Duly accomplished Oath Taking Form (with documentary stamp) - Php 250.00.
2. Current Community Tax Certificate.
3. 2 pcs. passport-size picture (scanned pictures are not acceptable, must be close-up and colored with white background).
4. 1 pc. short brown envelope.
5. Registration fee of P1,050.00.
