QUALIFICATIONS:

• Must have a master’s or Phd in a closely related field from a reputable university
• Passed the Licensure Examination for Teacher (LET)
• Must have at least 1 year of teaching experience
• Must have at least 8 hours of relevant training
• Preferably Registered Psychometrician or Registered Guidance Counselor for Psychology faculty applicants
• Preferably Certified Public Accountant (CPA) for Accountancy faculty applicants

1. Duly accomplished and Notarized Personal Data Sheet (PDS) (CS Form No. 212, Revised 2017) with recent passport-sized ID pictures and thumb mark affixed on the space provided together with the Work Experience Sheet which can be downloaded at

3. Photocopy of Transcript of Records;
4. Photocopy of Diploma;
5. Photocopy of Certificate of Training/s
6. Certificate of Employment; (if applicable);
7. Service Record; and
8. Resume (5 copies)
